Confidentiality
Confidentiality is an essential aspect of maintaining professional standards at WellQo, wellbeing service.  The service, WellQo, keeps all records secure and ensures confidentiality in the treatment of any information held about its clients, All information recorded and held is solely for the purpose of maintaining effective therapeutic treatment.  

What data is held and why?

Website

The information that is collected when you visit www.wellqo.co is your name, email, telephone number and any other information you wish to include when you submit a contact request.  I then use this information to respond to your request. 

Website cookies

Cookies are small files of letters and numbers that are downloaded on to your computer when you visit a website. When you visit www.wellqo.co, the site will gather the following information about visitors:  the number of visitors to the website, pages visited, new vs returning customers, average session time on website, and the country location of the visitor, in order to offer better site experiences and tools in the future.  However, you cannot be personally identified by visiting the website, unless you submit a contact request form, or request access to the newsletter using an email that includes your name, for instance.   

Other forms of data collected 

Contact data from the initial health assessment sheet, and information about the problem you would like help with, how it effects you, coping strategies used, and your goals for therapy, are kept on paper, and electronically, so that I can contact you, as well as decide on the best treatment and support options.  

I record information about what you tell me during sessions, usually making brief notes.  This is to help with applying therapy techniques in-session as well as to assess the progress of treatment over time.  

I also keep a record of the number of sessions conducted and payments made.

How is data stored?

All messages and information received via the website, such as name, email, telephone number and concerns, are stored on password protected devices.  Session notes are kept on paper, sometimes electronically,  and kept securely locked when not in use. Data held electronically is on a personal computer and stored locally in a password protected account.  Phone contact details are  held on a password protected work phone.

I record appointments on a password protected phone calendar, sometimes on paper in my diary, and data about the number of sessions and the payments you make on paper and electronically.  Your records are held for as long as is necessary for the purpose for which they were collected.

Upon written request by the client, all records will be erased within a 1 month period.  

Who is data shared with?

All information will be kept confidential unless the client and therapist agree to discuss something with a third party.  Exceptions to the confidentiality rule occur only in circumstances where the therapist/WellQo considers there to be a risk of harm to the client or other persons, or a requirement by law.  Where possible, any breach of confidentiality would be discussed prior.  

Access data

Upon verbal or written request, you may access your records.
